Use this form to describe your anticipated ministry activity during the 2024-25 Conference Year (ending in July, 2025). You are encouraged to reimagine ministry - (consider something new/different or innovative) and to collaborate, whenever possible. All ministries are required to complete this form. Submission of your ministry's preferred events, programs, and dates are all subject to approval. If a copy of this completed form is not forwarded to you automatically upon submission, please let the office know and we will send a copy as soon as possible.

ANNUAL MINISTRY BUDGET
Please provide your budget request in terms of importance with one (1) being the highest priority in budget request importance, followed by 2, 3, etc. The requests submitted will be considered by the committee; however should budget adjustments and cuts be needed, your prioritization of ministries/programs/functions will assist the committee in making the needed changes. The team will contact you regarding approval and/or changes to your budget. We understand that this list will not be exhaustive, but do your very best to be as complete as possible.
List your budget requests in order, by priority level, beginning with 1, 2, 3, 4, etc. (For example: 1. Annual potluck, $250; 2. Evangelism training, $150; 3. Senior prom, $100), etc. - THEN ENTER THE FINAL TOTAL of ALL requests.
