As a vendor working with the Texas Historical Commission, the State of Texas requires vendors to supply the following information to perform work with us. The business unit cannot proceed with a requisition or purchase order without these documents on file.

1) TINS Form
For information about the Texas Identification Number System (TINS), visit the State of Texas Comptroller office.
State payees include vendors who contract with a state agency to provide goods or services and individual recipients. In almost all cases, payees are required to provide one of these nine-digit identification numbers to obtain a TIN:
- Federal employer identification number (EIN)
- Social Security Number (SSN)
- Individual Taxpayer Identification Number (ITIN)
If the payee is a foreign entity or non-resident alien who does not have an EIN, SSN or ITIN, please check with the Comptroller office on how to request a Comptroller-assigned TIN.
International Vendors: You submit W-8BEN (entities) or W-8BEN-E (a foreign individual) instead of W-9 form.

- Complete the PDF and follow these guidelines on how to encrypt this form before attaching it below
- Once submitted, you'll receive an automated email with a new form link to submit your encryption password to the Accountant securely
2) W-9 Form
The Internal Revenue Service requires all vendors to provide the details listed in the W-9 form for tax purposes. Please visit the IRS here and download the W-9 form, complete it, encrypt it, and attach it to the form field below. The form has further instructions to assist you.
To submit the completed W-9 form to our agency, please follow these directions:
- Download the W-9 form from the IRS website as a PDF file
- Complete the PDF and follow these guidelines on how to encrypt this form before attaching it below
- Once submitted, you'll receive an automated email with a new form link to submit your encryption password to the Accountant securely
International Vendors: If you are located outside of the United States, you will submit a W-8BEN (entities) or W-8BEN-E (a foreign individual) form instead of the W-9 form. Follow the instructions to complete the form, save it as a PDF and encrypt it using these guidelines. Then, complete the form below and add your encrypted form in the W-9 attachment field.
