•I expressly agreed that participation in Chagolla Trained Wrestling Club (CTWC) and the use of CTWC and its facilities shall be undertaken by the parent/guardian at his or her risk.
•It is further agreed that Chagolla Trained Wrestling Club training center residences shall not be liable for any injuries or damages to parent/guardian, wrestler or other children with parent/guardian, nor the subject of any claim, injury, or damages, whatsoever, including but not limited to damages resulting from act of active or passive negligence on the part of Chagolla Trained Wrestling Club, its successors or its officers or agents.
•It is specifically agreed that Chagolla Trained Wrestling Club shall not be responsible or liable to the parent/guardian or wrestler for articles lost or stolen in the training centers. Chagolla Trained Wrestling Club shall not be responsible or liable for loss or damage to any other property of parent/guardian or wrestler including their automobile and contents thereof.
•It is also agreed that any damages to the training center facilities or property of any parent/guardian or wrestler by any other parent/guardian or wrestler, is the sole responsibility of the offending parent/guardian or wrestler.
•I understand that photographic, video and/or other images of my child may be obtained and used in the media, on the Chagolla Trained Wrestling Club Facebook ,Instagram website, or in other Chagolla Trained Wrestling Club promotional and publicity materials at the discretion of Chagolla Trained Wrestling Club, and that if I do not wish for my child's image to be used for these purposes I must request so in writing.
•I expressly agree that this release, waiver and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Arizona. This release is an agreement between the parties. This agreement and the terms of this release are contractual and not a mere recital.
•I give permission for my son/daughter to participate in Chagolla Trained Wrestling Club.
•I understand that there will be NO REFUNDS given after registration into Chagolla Trained Wrestling Club. I also understand there are no exceptions to this no refund policy. Should I, and/or my child/wrestler choose not attend Chagolla Trained Wrestling Club after registering, it will be done so with acknowledgment that all registration fees will be forfeited.
•I understand I will still be charged for the month I CANCEL my monthly membership if you choose to cancel.
•I certify that I am the managing conservator of this child or children, am authorized to sign this waiver, and accept the terms of this Agreement.

IMPORTANT: The $85 monthly pre-registration rate is available through August 25. Beginning August 26, the regular monthly rate is $95.
TOURNAMENT PARTICIPATION
CTWC's goal is to provide frequent competition and development opportunities, with a goal of at least two tournaments per month. A separate attendance post will be made for each event so coaches can know participation numbers and properly delegate coaching and athlete supervision.
TEAM EXPECTATIONS
☐ A CTWC team meeting will be held prior to the season to review expectations, schedules, communication, tournament participation, and team responsibilities.
☐ Parents/guardians are expected to communicate tournament attendance through the designated CTWC event post.
☐ Wrestlers are expected to represent Chagolla Trained Wrestling with discipline, respect, effort, and good sportsmanship.
☐ Training is designed to develop wrestlers on and off the mat through fundamentals, repetition, discipline, and character development.
PAYMENT & AGREEMENT
By signing below, the parent/guardian acknowledges the registration information, monthly membership rate, season dates, practice schedule, and team expectations listed in this form. Payment arrangements and any additional club policies are subject to the official CTWC registration agreement.
