The following form will be reviewed by a designated ombudsperson who will keep all responses confidential. You are welcome to submit the form anonymously; in which case, please provide an anonymous email address (e.g. see this article to create an anonymous email address) if you wish to be involved with/receive follow-up responses.
SCAR encourages the use of this form as a last resort, when all other approaches to address the issue in question between the persons involved have failed. Please check the Code of Conduct for other available support and resources.
