Annual Pilgrimage to Lourdes 2026 (28th April

This Registration Form is the application for the Annual Lourdes Pilgrimage 2026, an International Event for us to pray for the sick and disabled people together. It must be COMPLETED AND RETURNED BY 9th January, 2026.

INSTRUCTIONS. PLEASE NOTE THE FOLLOWING:

* Every person attending the Lourdes Pilgrimage ("the Pilgrimage") is required to complete this form and all data must be filled in. You will see the "PRINT" button at the last page and please print the completed form and then press "SUBMIT" button.

* The form must be received by the deadline above.

* The term "Organizer" means The Hong Kong Association of the Order of Malta Limited, organizing Lourdes Pilgrimage, a company limited by guarantee with a registered charity file no. 91/17181 and is exempt from tax under Section 88 of the Inland Revenue Ordinance.

* Useful notes for completion of the Application Form:

Terminology used in the form:

1. Volunteer=A volunteer will share the responsibilities to help and assist the Pilgrimage's malades (from Hong Kong or other countries) and be available for other tasks during the pilgrimage.

2. Pilgrim=A pilgrim who join all the spiritual and leisure and events and travel together with the group from the Hong Kong Association of the Order of Malta.

3. Malade=A pilgrim who has an sickness and/or disability, that makes it difficult for him or her to travel on a Pilgrimage to Lourdes without extra assistance.

4. Priest(s)=a spiritual leader who conducts ceremonies and prayers, and ensures the proper conduct of religious practices.

5. Religious team=a church member who leads prayers and rosaries, and ensures the proper conduct of religious practices.

*The processing of the data shall be carried out with the assistance of computerized instruments and on paper. The data shall only be accessable to the Organizers and staff of the Pilgrimage, and to any other persons as deemed necessary by the Organizers.

* Please note that all volunteers will be required to attend a 1-day refresher Home Nursing Training course in March/April. Date to be further confirmed in due course.

Section 5: Pilgrimage Fee
Each attendee is required to pay a non-refundable Pilgrimage fee of €200 or HKD2000 - this fee covers the local transportation, candles, tax in Lourdes and donation to sanctuary. When you submit this Registration Form, please deposit the payment to ICBC (Asia)Bank HKD Current Account 721-502-00720-4 (Account Holder Name: "The Hong Kong Association of the Order of Malta Limited") or FPS (ID No.:116360140). Please email or whatsapp the deposit slip to info@orderofmalta.org.hk or (852) 91616601.

We kindly ask for your understanding that without proof of payment, we will not be able to consider your application.

Attendees are required to arrange
- Visas
- Travel insurance
- Your passport which is valid for at least 6 months prior to the date of departure from Hong Kong
The Order will co-ordinate:
- Group air tickets
- Local Transportation such as shuttle bus
- Hotel bookings in Lourdes (the room rates are for reference only, subjected to change)
- Ladies' Uniform/Men's Uniform/Medals/Men's tie
- Program and activities during the entire Pilgrimage
