    First Reconciliation/ First Communion Registration Form

  • If you have a student that would like to make the Sacraments of First Reconciliation & First Communion during the 2025/26 faith formation year, please fill out the following form completely. All information is required for our records. 

    There is a mandatory parent meeting on for all families with children wanting to make their sacraments this year. There will be 2 sessions available, both sessions will be held in the Cathedral social hall.

    Session 1 - Tuesday, October 21st @ 6:00 PM

    Session 2 - Wednesday, October 22nd @ 8 AM

  • All students receiving any Sacraments must have a Baptismal certificate on file. All Baptismal certificates are due to me before November 15th. 

    If your student was Baptized in one of our Duluth parishes or has one on file at Stella Maris Academy, I will get the certificate for you.

    If your student is not Baptized and you would like them to receive all three sacraments this year please email me as soon as possible.

  • Students and their families are required to uphold our Sunday Mass obligation. If the priests in our Mission Field do not recognize you and your famiy as regularly attending Sunday Mass your child will not receive their Sacraments.

    As stated in the Catechism:

    "The Sunday Eucharist is the foundation and confirmation of all Christian practice. For this reason the faithful are obliged to participate in the Eucharist on days of obligation...Participation in the communal celebration of the Sunday Eucharist is a testimony of belonging and of being faithful to Christ and to his church"

  • I consent to the participation of my child/ward in the above named activity. In consideration for my child/ward's participation, I agree to reimburse and indemnify East Duluth Faith Formation(understood to include the Diocese of Duluth, Stella Maris Academy Holy Rosary Campus, Cathedral of Our Lady, St. John the Evangelist, Saint Benedict Parish, Saint Mary Star of the Sea Parish, Saint Michael's Parish, St Joseph's Parish) for all reasonable legal and court fees incurred by East Duluth Faith Formation in defending a lawsuit that I or my child/ward may bring against East Duluth Faith Formation which relates to the above named activities. If East Duluth Faith Formation is found not legally liable By the courts and prevails in the lawsuit. If East Duluth Faith Formation is found liable for the injuries sustained by the child/ward, this paragraph will not apply.
    I certify that I have an understanding of this agreement and the risks and hazards associated with the activity described above that my child ward will be participating in. I further understand that I had the opportunity to fully discuss this agreement with a representative of East Duluth Faith Formation to clarify any concerns or questions about the activity or this agreement that I may have had.
