First Reconciliation/First Communion Registration Form

Welcome to the sacramental preparation journey for First Reconciliation and First Holy Communion! This significant milestone marks a beautiful chapter in your child’s spiritual life as they prepare to experience God’s healing mercy in First Reconciliation and receive the True Presence of Jesus for the first time in First Holy Communion.

There is a brief parent informational meeting for families on Wednesday, September 30th at 9 AM in the Cathedral social hall and 7:00PM in the Cathedral multipurpose room.

All students receiving Sacraments this year must have a Baptismal certificate on file. All Baptismal certificates are due to me before November 11th.

I understand that all those in the Catholic church who have reached the age of reason must uphold the obligation to attend weekly Sunday Mass and on Holy Days of obligation. I understand that it is required of all First Communion students and their families to attend weekly Sunday Mass. I undertand that if this obligation is not met, my student will not be able to receive their sacraments this year.

I understand that all important information and dates for First Reconciliation and First Communion can be found on the First Reconciliation/ First Communion page linked at the top of this form and can be referenced all year.

I understand that it is my responsibility to provide a copy of my child's Baptismal certificate to the faith formation program by 11-11-26. Details on this can be found on the First Communion page.

I understand that there is a required parent formation session for First Reconciliation on Wednesday, November 4th from 5: 30 - 6:30 PM in the Cathedral.

I understand that First Reconciliation is on Saturday, December 5th, at 9:30 AM at the Cathedral for all students. I also understand that parents stay with their student for First Reconciliation.

I understand that there is a required First Communion retreat on Saturday, April 24th from 9 AM -10:30 AM. I also understand that this retreat is required for both parents and students.

I understand that First Communion rehearsal on Saturday, May 1st is required for both parents and students. Details on times for each parish can be found on the First Communion page.

I understand that First Communion is on Sunday, May 2nd and details can be found on the First Communion page.

I understand that communication for First Reconciliation and First Communion will come through the faith formation program through Flocknote. I understand that if I am not receiving the weekly emails I will let Sonya Morris know.

I understand that if these expectations are not met that my student will not be able to receive their Sacraments this year.

I hereby grant permission for my child ward to be photographed and or videotaped during the retreat. I understand that the resulting photographs and or video tape footage may be edited, if necessary, and then published and or posted in parish newsletters, website, and other material associated with the East Duluth Catholic Faith Formation ministry.

I consent to the participation of my child/ward in the above named activity. In consideration for my child/ward's participation, I agree to reimburse and indemnify East Duluth Faith Formation(understood to include the Diocese of Duluth, Stella Maris Academy Holy Rosary Campus, Cathedral of Our Lady, St. John the Evangelist, Saint Benedict Parish, Saint Mary Star of the Sea Parish, Saint Michael's Parish, St Joseph's Parish) for all reasonable legal and court fees incurred by East Duluth Faith Formation in defending a lawsuit that I or my child/ward may bring against East Duluth Faith Formation which relates to the above named activities. If East Duluth Faith Formation is found not legally liable By the courts and prevails in the lawsuit. If East Duluth Faith Formation is found liable for the injuries sustained by the child/ward, this paragraph will not apply.
I certify that I have an understanding of this agreement and the risks and hazards associated with the activity described above that my child ward will be participating in. I further understand that I had the opportunity to fully discuss this agreement with a representative of East Duluth Faith Formation to clarify any concerns or questions about the activity or this agreement that I may have had.
