Please NOTE - Day School Training Application is specifically for our Day School Training program.  These are typically 6 week classes and have various Monday start dates.  A Canine Crews representative will follow up upon receipt of application to determine class, start time and payment.

My dog must pass a temperament test to ensure they are suited for our off-leash playgroups and handlers.

I understand that the degree to which a dog is successfully trained is a function of the cooperation of the dog.  I therefore acknowledge and agree that there is no guarantee that my dog will achieve the desired level of training despite the best efforts of the trainer. 

I understand that once the dog training session for which I registered has begun, there are no refunds.

My dog's complete veterinary inoculation records must be furnished to Canine Crews including rabies, Distemper Combo (DHLPP), bordatella and a negative fecal exam for parasites. I also understand that Canine Crews recommends the canine influenza vaccine.

My dog must be spayed or neutered unless younger than eighteen (18) months of age unless joining our ACE program. If we notice any behavioral challenges with unaltered dogs under that specified age in our playgroups, a behavioral conversation will be had regarding their continuation or removal from the playgroup.

I understand that socialization far outweighs the risks and know that participating in off-leash group play exposes my dog to various airborne and nose-to-nose contagions.  Including, but not limited to:  canine cough, giardia, upper respiratory infections and papilloma virus.

I understand that I am required to communicate any known illness in my own dog(s) at any time to Canine Crews to allow for full disclosure to all clients regarding illness.

Client Agreement and Release of Liability

I hereby release Canine Crews LLC, its agents, officers, sub-contractors, employees, animal owners, customers, and potential customers of Canine Crews LLC from any and all liabilities, financial, and otherwise for injuries to myself, my dog, or any other property of mine, which arise, in any way from services and/or products provided by or as a consequence of my association with Canine Crews LLC. I agree to assume all liabilities and responsibilities, financial and otherwise, for the behavior and health of my dog. In consideration of the services rendered by Canine Crews LLC, I waive any and all claims, actions, or demands of any nature, foreseen or unforeseen, that I may have against Canine Crews LLC relating to the care, control, health, and/or safety of my dog arising during pick-up, transport, drop-off, and stay at the facilities. I authorize Canine Crews LLC to do whatever they deem necessary for the safety, health, and well-being of my dog while under the care of Canine Crews LLC, including seeking professional veterinary treatment for my dog. I understand that if my dog has a history of or repeatedly demonstrates aggression or biting of humans or animals, Canine Crews LLC reserves the right to refuse service. I understand that all bites will be reported to the local authorities, as required by law. I hereby declare to Canine Crews LLC that I am the legal owner of my dog; that my dog has not been exposed to distemper, rabies, or parvovirus within the past thirty (30) days, and that my dog has been inoculated as indicated by records presented.

Payments are due at start of Day School Training Classes.  Payments for daycare & boarding are due at end of day. I understand that the hours of operation at Canine Crews LLC are subject to change at any time. Late pick up without communication is not allowed. If I have not picked up my dog by 1(one) hour after posted closing time, Canine Crews LLC will board my dog overnight at my expense. Should this take place, I authorize in advance that Canine Crews LLC will automatically charge my credit card a $25 late fee, in addition to boarding and daycare fees. I authorize Canine Crews LLC to charge my credit card for any outstanding balance on my account. I understand that I will be charged a $25 handling fee for returned checks. By signing below, I acknowledge that I have read this agreement in its entirety and agree to the terms. This agreement shall be binding for a period of ten (10) years from the date of signature below.

Canine Crews LLC reserves the right to use any all photos of client dogs for marketing and PR purposes.
