We would be happy to provide you with an estimate of our services for your needs. Please use this form to provide us as much accurate information as you can. Our Veterinary Technicians will use the information you provide to create an estimate.

• In this document we will request the most recent records (up to the last 3 years) of your pets medical history. (If you experience problems uploading your documents, you can email them separately using the email, info@glvcvet.com)
• Please fill out one form per pet.
• Once we have received the completed form along with the records, please allow us 3 business days to respond with an estimate. (Be sure to check your spam or junk email for your estimate.) If you have not heard from us by the 4th day please give us a call.
