  • Contract Overview:

    Unconscious Beaute' Studio is committed to delivering the services specified in Section 2, as requested by the Client. In return, the Client agrees to engage Unconscious Beaute' Studio for these services at the cost outlined in the "Service Agreement Guidelines."

    The provision of services by Unconscious Beaute' Studio will proceed as follows:

  • Service Agreement Guidelines:

  • 1. Payment Schedule:

  • Payment Conditions: Clients paying privately or through insurance are required to remit the minimum deposit and complete payment for services within 45 days from the contract signing date with Unconscious Beaute' Studio. For clients utilizing Veteran benefits, the deposit will be charged upfront, and the remaining balance will be invoiced to the VA upon product completion. All deposits are non-refundable.

  • 2. Cancellation Policy:

  • Cancellation Terms: Should the client need to fully cancel the order before making the final payment, a refund of 30% of the total product cost will be issued as stipulated in the contract. For veterans who cancel prior to the arrival of the Cranial Prosthesis, the payment will be refunded to the VA Hospital.

  • 3. Shipping Guidelines:

  • Cancellation Terms: Should the client need to fully cancel the order before making the final payment, a refund of 30% of the total product cost will be issued as stipulated in the contract. For veterans who cancel prior to the arrival of the Cranial Prosthesis, the payment will be refunded to the VA Hospital.

  • 5. Revised Payment Schedule:

  • Payment Rescheduling Terms: The CLIENT agrees to provide at least one week's notice for any rescheduling of payment due dates. Any changes to the schedule or payment amount must be promptly communicated, and confirmation of such changes must be received from the COMPANY by the CLIENT.

  • 6. Additional Provisions:

  • Client-Company Cooperation: Both the CLIENT and the COMPANY acknowledge that timely cooperation is crucial to meet the objectives outlined in the contract. The CLIENT is responsible for clearly communicating all styling preferences, such as parting, style, length, color, and any special customization requests. It is also the CLIENT's responsibility to inform Divine Beauty Bar of any changes or issues within 90 days of receiving the unit.

  • 7. Obligations:

  • Payment and Shipping Limitations: The COMPANY is not liable for compromised payment schedules due to factors beyond its control, including but not limited to, insufficient funds, incorrect product orders, late payments, adverse weather conditions, scheduling issues, or incorrect addresses provided.

    The COMPANY assumes no responsibility for adverse conditions affecting the product during shipping, nor for products misplaced after being signed for upon delivery. The COMPANY is not accountable for any failure in product receipt or refusal at the time of delivery and will not issue refunds due to client refusal upon delivery.

  • 8. Liability Limitations:

  • Contingency Clause: Should an unforeseen circumstance such as injury, illness, natural disaster, or any other event beyond the control of the COMPANY prevent Unconscious Beaute' Studio from fulfilling the terms of this contract, every effort will be made to secure a suitable replacement. If no appropriate substitute can be arranged, the extent of the COMPANY's responsibility and liability will be limited to the refund of all payments made for the Product(s).

  • 9. Image Usage:

  • Photo Release Consent: Clients are required to fill out our Photo Release Form. Upon signing, Unconscious Beaute' Studio reserves the right to use these images on all social media platforms for marketing and educational objectives.

  • 10. Refund Policy:

  • Compliance and Health Precautions: In accordance with skateboard regulations and health considerations, no refunds or exchanges will be permitted on any products. All sales are final.

  • Signatory Acknowledgment: The parties below have read, understood, and consent to all terms and conditions laid out in this contract.
