Volunteer Agreement: This information enables us to keep an integral environment. This form will be kept confidential. Thank you for your willingness to volunteer. By completing and signing this form, you give Music2ChangeU permission to submit your information for a pre-background check to ensure the safety of our students. As a volunteer, I agree to: Take all reasonable steps to protect myself and see to the safety of others. Declare that I am a fit and proper person of good character. Allow our organization to keep your personal information on file for youth ministry use only. Ensure I am appropriately dressed, and that none of my attire is offensive.

It is the express intention of the Parties, and the Parties hereby acknowledge, that the volunteer is unpaid and not an employee or partner of Music2ChangeU, SpiritFest, ERay Promotions, Change Church, Bon Secours Wellness Arena, Greenville Arena District, or New Nazareth Church. Nothing in this agreement shall be interpreted or construed as creating or establishing a relationship of employer and employee. B.) Volunteer further acknowledges and agrees that volunteer may not covered by Music2ChangeU, SpiritFest, ERay Promotions, Change Church, Bon Secours Wellness Arena, Greenville Arena District, or New Nazareth Church insurance and hereby releases entities, board members, representatives, and agents from claims of injury incurred during any and all any rehearsals or performances, and events. C.) Volunteer acknowledges and agrees that any and all services provided under this agreement are provided on a voluntary basis and that no compensation or benefits of any nature shall be due and owing from Music2ChangeU, SpiritFest, ERay Promotions, Change Church, Bon Secours Wellness Arena, Greenville Arena District, or New Nazareth Church for any service.

My signature signifies my understanding this volunteer agreement and that I have answered each question honestly and truthfully. I certify that the answers herein are true and complete to the best of my knowledge. I authorize the use of the above information for the sole purpose of maintaining a safe environment for youth and an organization of integrity.

As a volunteer, I agree, Music2ChangeU, SpiritFest, or ERay Promotions has the right to use my picture, ideas, content, silhouette and other reproductions of likeness, music and voice in connection with marketing campaigns, reels, documentary or for media material and promotions. Music2ChangeU, SpiritFest, or ERay Promotions shall have all right, title and interest in any all results from said advertisement’s images or appearance unless otherwise opted out in writing. Rights are perpetual and include the use of any and all interviews, performances, and recordings in any medium, video, pictures or images.
