St. Michael Catholic School Sports Supervision & Transportation Registration 2025-2026
We are pleased to offer students supervision and transportation to the STMA Middle Schools so that they may participate in afterschool activities. In order for students to use this service, they must be registered each season. To register, complete this form and either pay the registration fee electronically below or bring the registration fee to the school office. You are not registered until we receive the form AND the fee.

This form must be completed and turned in 3 school days before transportation is needed to ensure adequate staffing.

If your child would like to ride the bus over for an eligibility meeting, an email request must be sent at least one day before the eligibility meeting to shouse@stmcatholicschool.org.

We plan our staffing based on this form. If practice changes, a game is added, or the season is extended for any reason, parents or students must let us know at least the day before to make sure we have staffing in place.
Payment - Cost per season is $25.00. This fee will help cover our staffing cost for the supervision of the classrooms before the transportation bus arrives.

I would like to pay the registration fee by credit card now.I would like to pay by cash or check. I will bring that to the school office clearly labeled as "Sports fee" with the student's name. I understand the registration is not complete until the registration fee is received.
In the event of practice or game cancellations, please specify below what your student's plans should be for getting home. If they will be picked up by another family, please choose the "other" option and specify who they have permission to ride home with.
