Our mission at Math Circles of Chicago (MC2) is to create opportunities for all students in Chicago to develop a passion for mathematics.
MC2 conducts ongoing internal evaluations to help us understand if we are accomplishing our mission, to identify areas for improvement, and to better communicate our impact to school officials, donors, and the community.
This particular evaluation will use student data to determine the impact of our education enrichment program on student academic achievement and attendance. Part of this analysis will be done utilizing student information from Chicago Public Schools (CPS If your student is not in CPS, this consent form does not pertain to you.
With your permission, we would like to access the following information from your child's CPS school records from these academic years: 2022-2023 (4th Grade), and 2023-2024 (5th Grade)
- IAR (Illinois Assessment of Readiness) Math scores
- Cumulative Math grades and/or end of year transcripts.
- Attendance Information
This data will not be associated with your child's name and we will never have access to their name.
Rather, the data will be associated with your student's CPS student ID number. We will use this data to examine associations between participation in MC2 and student outcomes and the results will be included in reports for MC2.
These reports may be used internally for program evaluation or communicated externally to the community, the board, and existing/potential donors.
The reports will only provide aggregated data and no individual data will ever be shared with third parties.
Student information will remain strictly confidential and will only be used by MC2's staff. Strict guidelines are in place to ensure confidentiality and each MC2 staff member who will have access to the data has received appropriate training. At no point will individual student data be publicly released.
The only risk to your child would come if confidentiality is breached. We store this information on secure servers and make every effort to protect it.
You may withdraw your permission at any time by notifying MC2 in writing.
We hope you will agree to help MC2 by participating This will provide us with valuable information on how to improve our work and communicate these results to our community.
However, if you decide to withdraw your permission, you may do so by notifying us in writing at 1034 N. Wells, Chicago, IL, 60610, or via e-mail at info@matheirclesofchicago.org.
Your decision to provide us with access to your child's data will in no way affect their ability to participate in MC2 programming.

If available, please provide a CPS student ID number for your student. The student ID number can be found on your student's report card or student ID card. Please make sure you record the 8-digit ID number on the form accurately.
Parents, please be aware that under the Protection of Pupil Rights Act. 20 U.S.C. Section 1232(c1A), you have the right to review a copy of the questions asked of or materials that will be used with your students.
