Furiend Flight is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray (satos), neglected and abused animals. Our mission is to provide animals with essential vet care, vaccines, spaying/neutering, TNR or medical treatment. Once ready we help arrange transportation from Puerto Rico to our fosters and adopters in Arizona and nationwide, and at no additional cost(s). Furiend Flight is completely funded through fundraising efforts and public donations. All of our pets are cared for in foster homes settings, as we believe that animals are pets and made to be apart of our family.

Please read the below statements in it's entirety and mark "AGREE" before proceeding with the pre application 1.The undersigned (“Adopter”) acknowledge and agrees that Furiend Flight has reviewed and approved my request. This form serves as my final Adopter and Furiend Flight interview. FF and Adopter have been in contact via social media, text (sms) messages via a phone call, emails or live feed video formats (such as Facebook Messenger Video, Whats App, FaceTime or Skype). a. b. During the Adoption Application process I full understand that pets, much like children, require guidance and although FF does their best to set all their pets up for success with their new adopted families, enlisting the help of reputable trainers is strongly encouraged when needed. c. Within the Application process, I understand the measures that I the Adopter should take when welcoming a new pet into my home, including, but not limited to: i. Decompression time of a minimum of 3 days to see pets personality, 2 weeks to start feeling truly comfortable and safe.; ii. Introduction steps for the new addition with current pets within the home; iii.Not overwhelming the dog with dog parks, house guests, etc within the first 2 weeks; iv. Establishing care with Adopter’s local vet within 60 days of arrival ; and v. Verifying Microchip has correct contact information and remains up to date throughout the life of the pet *

ADOPTION FEE: Furiend Flight requires an adoption donation of $400 for US based canine adoptions (where pet was previously flown out from the island). Feline adoption donation $225 for 1 or $180 for a pair (2) felines. For LOCAL Puerto Rico Adoptions a $200 canine or $125 feline donation for adoptions (no flight required). This donation fee which goes directly to our rescue efforts in Puerto Rico and helps pay for the food, boarding, veterinary care, medications, and airline transport of our dogs to the mainland US. If the pet you are applying for is too young to be spayed or neutered before a foster-to-adopt agreement (where the adoption is not finalized), will be in place until the pet is of age to be fixed. At that time the pet will be spayed or neutered by Furiend Flight appointment or by Vet Certificate Voucher from Furiend Flight and the adoption will then be finalized. Please note: on average, we spend $500 and up on each dog or cat we rescue to ensure their health and safety. Your adoption plays a critical role in allowing us to continue rescuing and rehabilitating abandoned dogs and cat in the future.*

2. If adopting a pet (canine)with a working or herding breed background in it's DNA (such as but not limited to shepards, collies, pointers, hounds, labs, retrievers, terriers etc: a.Adopter is aware that these dogs typically need more guidance, regiment, and purpose and without such can exhibit destructive, undesired behaviors. Adopter will contact the rescue partner immediately and seek help from a reputable trainer should concerns arise; b.Adopter is aware that working or herding breeds tend to be more mouthy and nippy and have a tendency to exhibit herding behaviors with other animals and potentially humans if not properly trained; and c.Adopter is aware that working or herding breeds can be more prone to a strong prey drive. *

3. I hereby agree to care for the above-described pet in a caring, humane and responsible manner and to provide it with clean and adequate shelter, food, water and veterinary care. I hereby agree said animal will mainly be indoor pet that WILL NOT be tied up or tethered outside for ANY REASON! I further agree that said pet shall reside inside my home and shall not be allowed to roam freely. I hereby agree that this pet shall wear a collar or harness with identification tags at all times. I hereby agree that the adoption fee of a minimum of $200.00 helps to cover the pet's current expense of raising and caring for the animal and is not refundable. I hereby understand and agree that Furiend Flight makes no representations or warranties, expressed or implied, about the abovementioned animal's temperament and is hereby absolved from any liability for future damages or injuries caused by said animal. I the Adopter also understand, acknowledge, and agrees that any and all donations paid by Adopter in connection with the adoption contemplated hereby, including any additional costs, such as boarding, flight or transportation fees, will be requested, and are NON-REFUNDABLE.*

4. Adopter acknowledges and agrees that if, for any reason, the Adopter is unable or unwilling to keep the pet and provide the pet with the care necessary or advisable for its health and well-being, the Adopter shall immediately contact Furiend Flight to return the pet to the Rescue Partner, at the sole cost and expense of the Adopter, before making any other arrangement with respect to the housing of the pet. The Rescue Partner and/or Furiend Flight will notify the Adopter if either of them has determined that it is able to take possession and/or secure alternative placement of the pet. Before such time as Furiend Flight and/or the Rescue Partner takes possession or places the pet with an alternative home, the Adopter agrees to maintain the health and well-being of the pet, either in the Adopter’s home or at a safe and professional boarding location. The Adopter agrees that it shall not sell, move, gift, or otherwise transfer the pet to any other person (including any family member or shelter) without the prior written consent of Furiend Flight or the Rescue Partner.*

5. Adopter acknowledges and agrees that they have had an opportunity to ask questions regarding the adoption contemplated hereby. Adopter acknowledges and agrees that none of FF Related Party (as defined below) has made any representations or warranties regarding the subject pet, the adoption contemplated hereby, or any related matter, and Adopter is not entitled to rely on any representations and warranties of FF or any FF Related Party with respect to such subject matter. Adopter, on behalf of itself, its successors, heirs and assigns, hereby generally, irrevocably, unconditionally and completely releases and forever discharges FF, its affiliates, and its and their directors, officers, managers, employees, agents and advisors, and each of their respective successors and assigns (collectively, “FF Related Parties”) from all disputes, claims, losses, controversies, demands, rights, liabilities, actions and causes of action of every kind and nature, whether known or unknown, arising from any matter concerning the subject pet, the adoption contemplated hereby or any other related matter or agreement between Adopter and the Rescue Partner and/or FF, or any matter contemplated by or related thereto.*
