Adult Personal Information Release Agreement

Without compensation, I hereby grant permission to Catholic Social Services to use and reproduce all content (text, photos, videos, documents) contained within this form submission. This content may be used for news and editorial purposes in publications, electronic reproductions (web sites), and/or brochures. In addition, I grant my permission to alter any submitted photos or videos without restriction and to copyright the same.

I also grant permission for release of information provided during an interview with CSS or submitted through this form for use in publications including videos, stories, and articles.

I hereby release the photographer, the journalists, and the publications or media outlets they represent, as well as Catholic Social Services, from all claims and liability relating to said content provided.
