By signing this form, First Name Last Name ,agree to the order I have placed and the fees stated. I understand that I will pay 50% down at time of drop off. I understand that I will be contacted by taxidermy for final instructions. At which time final balance is due for mount to be finished.
Pick-Up: I understand I will be charged $20/week for storage fees after 60 days of mount completion until taxidermy is picked up. (Storage fees apply) after a year mount will be sold.
Shipping: Taxidermy will NOT be shipped until full invoice is paid. I understand I will be charged 10% of invoice/week, for storage fees, until the next shipment date. (Storage fees apply)
