The services being performed at{name} are being performed by Estefania Soto for the purposes of hands-on training and/or education.

In consideration of the services the client will receive the model agrees as follows:

I agree to participate as a model for the following specified service: lash extensionsI understand that the beauty services I am receiving are being performed by a new licensed professional for the purpose of hands-on training and/or education.I understand that while Estefania Soto will do the best work, specific end results are not guaranteed.I understand there are certain risks involved with participating in a training program included but not limited to: improper application and/or technique in the service(s) provided, possibility of future correction and/or need for additional services, extended services times, adverse reactions and/or injury that may be sustained during such services. Dissatisfaction of services rendered.I agree not to hold either responsible for any risks involved in the participation of this training program.I agree to release and discharge the above-mentioned school/training program, or its management, employees, or students, individually or collectively, and its respective representations, licensees, successors, assignees, and its clients, from any and all claims, actions and demands now known or later discovered, including, without limitation, any and all claims for personal injury and other risks listed herein arising out of or in connection with the participation as a student model and the services received.I am over the age of 18 and have the legal capacity to sign this release. I have read the above and fully understand the contents. This release is binding upon me, my heirs, and legal representatives.I agree that by selecting the “Submit” button i am signing this agreement electronically. I agree my electronic signature is the legal equivalent of my manual/handwritten signature in this agreement.

By signing below I acknowledge that I have read the foregoing Model consent Liability Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Model Consent Liability Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
