Affiliate Program & Data Capture Form

Affiliate Program Registration / Data Capture Exemption Form
This form is to be used by NSWTA Affiliates to request the exclusion of the $30 TFA National Membership Fee and the $30 NSWTA Membership fee from MySideline registrations for Affiliate Programs, Data Capture or Payment Gateway functions. Examples below:
- Affiliate Program - Come N Try, School Holiday Clinic, Tiny Touch
- Data Capture - Expression of interest for team/event
- Payment Gateway - Representative payments, uniform or merchandise purchase
Please note this form does not apply for school based teams participating in your local competitions.

Before submitting this form, please ensure your Association has set up the "competition" (ie Program Name, etc.) in MySideline.

Please allow 2-3 business days for NSWTA/TFA processing. You will receive a notification of successful set up directly from NSWTA.
