FINAL REPORT REQUIREMENTS
(PDF up to 5 pages including invoices - one-inch margins and 12 point Calibri font)
Must contain:
1) A statement that confirms the “Scope of Work” described in the Grant Contract – Exhibit A; how it was executed, and how these funds helped the company advance its technology/business growth;
2) Relevant INVOICES supporting the activities outlined in the SOW to be paid by the grant;
3) Relevant reporting and documentation that supports the IMPACT of the contracted activity; i.e., specific outcomes of the grant-funded work described in both a quantitative and qualitative fashion, and other outcomes achieved through the completed work;
4) Signatures from the a) MBIA Member Center Leadership; b) Original Grant recipient / Company; c) Contracted Vendor.
