          This Agreement is made on this date:

          1. Service Provider: Santa T Glen Burnie MD

          2. Client: To whom is signing this form 

          3. Location: Service Provider travel fees based on distance from Glen Burnie, MD. Starting at 20.00, will be charged separately through the square link. 

           

          Services Provided: The Service Provider agrees to provide character entertainment services as detailed above once approved, and the date is guaranteed.

           


          Compensation:

          Total Fee: based on products you have chosen from above with approval 
          Payment Terms: Paid in full at the time of booking

          Cancellation Policy: If Santa can not make a booking due to sickness, we will set a new date, or the client can request a refund.  For any changes due to weather or natural causes, the client can request a new date or partial refund. 


          Client Responsibilities: Please call ahead of time with any changes to the date or venue. Any changes to the date or time will only be approved based on Santa's availability. No refund is guaranteed!

          Liability:

          The Service Provider shall not be liable for any damages or injuries caused during the event. The Client agrees to indemnify and hold harmless the Service Provider from any claims.
          Force Majeure:

          Neither party shall be liable for any failure to perform due to unforeseen circumstances such as natural disasters.
