Enrollment Disclosure
Liability Waiver:
I hereby state that my child is in good mental and physical health and is able to participate in all activities and classes. I hereby release GEM Multicultural Center LLC, doing business as GEM Academy & Center (“GEM Academy & Center”), and its respective agents, staff, mentors, instructors, volunteers, and vendors (each, a “Releasee”) a from any and all liability, claims, (including without limitation but including, all liability, suits, demands, judgments, costs, interest, and expenses, (including attorney fees and court costs) arising from, or related to, such activities and/or any related activities — including without limitation academic instruction, project-based instruction, and specialized classes such as Physical EducationicatPE and Robotics — and any accident or injury to the student and the costs of medical services, in each case, arising from the ordinary negligence of any Releasee. I understand that specialized classes may involve physical activity or the use of equipment, and I consent to my child's participation with this understanding. I also agree not to hold these parties responsible in the event that I or my son/daughter engages in inappropriate conduct (including, but not limited to, disruptive or volatile behavior in or out of the center) or becomes involved in any activity or with any persons not associated with this program. I further attest that the information in this application is correct to the best of my knowledge.
Media Consent:
I hereby allow myself/child to be photographed and videotaped at GEM Center for the purposes of the Center’s promotional material and publications.
Academic
Refund Policy:
All amounts paid shall be nonrefundable except in accordance with the terms of the Refund Policy linked here: https://www.gemcenters.org/refund-makeup-policy
Makeup Class Policy:
We do not offer makeup classes for illnesses, including COVID-19, or for bad weather that results in district closure or unannounced absences.
Promo Code Policy:
All promotions and coupons are active for three months after registration.
Cancellation Policy:
Cancellable for future months only with a written 14-day notice to admin@gemcenters.org
Exit Interview:
You agree to an exit interview with the tutors and Academic advisor before canceling or opting out of the program.
Final SAT Scores:
You agree to share a snapshot of your child's official SAT scores before canceling or opting out, so we can evaluate our efforts and establish internal accountability.
Termination of Enrollment:
GEM Academy & Center reserves the right to terminate a student's enrollment at any time, at its sole discretion, for any reason whatsoever, including but not limited to: misconduct, disruptive or unsafe behavior, failure to comply with school policies, non-payment of tuition or fees, or conduct that negatively impacts the safety or learning environment of other students or staff. Termination for cause does not by itself entitle the family to a refund of any tuition or fees already paid. Any refund is made only in accordance with the Refund Policy linked above
