This is an appointment request form. Filling this out does not guarantee you an appointment. After submitting this form, you should hear from us within 1 week about scheduling an appointment. A $100 security deposit will be taken at time of booking to secure your appointment. This deposit is non-refundable, but does come off the price of your tattoo upon the completion of the project. If you need to reschedule you must notify the shop at least 48 hours in advance. Cancelling/rescheduling within 48 hours of your appointment will result in a forfeiture of your deposit, at the discretion of the artist. At that point if you want to reschedule you will have to leave another $100 deposit. This is standard for the vast majority of the tattoo industry and serves as protection of both our artist’s and your time and effort.* This field is required.Nobody under 18 will be tattooed, no exceptions.
