Supported Employment:
Offers funded individual’s competitive employment in the community with a high level of support. Individuals are placed into a wide variety of jobs based on their preferences, provided job coaching to ensure a successful transition, then provided with ongoing follow-up and support for the duration of their employment   Initials   

Supported Living:
Supported living is an opportunity for adults with developmental disabilities to choose where, how and with whom they live. People receive personalized supports needed to maintain their own private home. Through supported living, individuals, even those with severe disabilities can receive supports to live in their own homes. Initials   

Personal Support (Live in):
Provide 8 hours or more of service while living with consumer assisting with personal care, activities of daily living and other activities necessary to maintain the individual in his or her own home.   Initials      

Personal Support (hourly):

Respite:
