ISS Coordinator will fill out this form for all students sent out of the classroom or closed out due to being tardy to class without a pass.  This data will be used for Tier2/3 students who have difficulty with being on time and prepared for instruction.  It will also be an accountability tool for teachers for make-up work when instruction is missed due to missing students.
