• I agree to have basic contact information (parent & dancer names, phone number, & e-mail address) shared with other dance families for dance related communications only.
• I give permission for my dancer's photo to be used for Ashden promotional information. All photos will be deemed appropriate by instructor before being used.
• I further absolve the Ashden School of Irish Dance or any persons acting on their behalf from any accident, injury or loss of personal property incurred while on the premises.
• The undersigned gives permission to the Ashden School of Irish Dance, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.
• I understand and agree with the withdrawal policy: If you desire to withdraw your dancer from the school at any point, notice must be given in writing (e-mail is acceptable). Refunds will be granted for the full year if written notice is given before the 1st of October. Refunds for the second half (February-June) of the season will be granted if written notice is given before the 15th of January. Retroactive refunds due to injuries are not allowed, nor are refunds should a dancer opt not to attend several classes.
