Community Outreach Scholarship Application

Oklahoma Flute Society offers scholarships in the form of free memberships and free admission to events. These scholarships are designed to benefit students grades 6-12 who would otherwise not be able to take advantage of what OFS has to offer. They are based on teacher or band director nomination/recommendation, and are subject to approval by the board of directors. If you are a teacher (or band director) and would like to nominate a student (or group of students), please complete the following information.

Teacher/Student Committment:

I agreee to submit to the board a brief follow-up report on their experiences at the OFS events they attend. Both teacher and student must agree to submit these reports via the Feedback Booth before the end of April. It is the responsibility of the teachers to make sure theses deadlines are met.
