This form is to be submitted only by the student after the standard Withdrawal deadline as outlined on the Academic Calendar and submitted by the last day of the term in which the course(s) were taken. Otherwise, the request will not be approved.
TU Policy: In cases of extreme hardship and/or extenuating circumstances where an incomplete grade is not warranted, a student may petition for withdrawal by submitting the appropriate documentation and obtaining approval from the Vice President of Academic Affairs. The request must be submitted no later than the last day of the term in which the course(s) are taken. The student must have been passing the course at the time the hardship or extenuating circumstance occured. The student is financially responsible for the course.
