HOLIDAY ARTS & CRAFT FESTIVAL

VENDOR APPLICATION

INFORMATION ON SPACES

INDOOR SPACES are $50 each. You will be provided with one 6 foot table and 2 chairs for each spot. Your set up will be on and directly in front of your space not to extend past the edges from side to side. ONLY 10 INDOOR SPACES ARE AVAILABLE.

OUTDOOR SPACES are $25 each and measure approximately 10x10. You will be responsible for providing your own canopy (including weights in case of high winds) and any other materials/supplies for your set up. Spaces are on a first come/first served basis the morning of the event. No electricity is available. ONLY 20 OUTDOOR SPACES ARE AVAILABLE. These spaces will be directly in front of our building in our parking lot so parking for your vehicle will be provided elsewhere near the facility.

 

Once your application is submitted, you will receive an invoice via email for the space you requested.

YOU ARE NOT REGISTERED UNTIL PAYMENT IS MADE!

Once registered, approved vendors will receive an email with further details about the event.

ART  WAIVER & RELEASE  OF LIABILITY FORM
           I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES sponsored by the Midland Palette Club, associated with this event, including by way of example & not limitation, any risks that may arise from negligence or carelessness on the part of the Midland Palette Club, persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault. In consideration of my application & permitting me to participate in this activity, I hereby take action for myself, my art entries, my executors, administrators, heris, next of kin,  successors, and assigns as follows:

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to liability arising from the negligence of fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to the Midland Palette Club and /or their directors, officers, employees, volunteers, representatives, and 
(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from and and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.
I acknowledge that Midland Palette Club and their directors, officers, employees, volunteers, representatives, and agents are NOT responsible, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.

I hereby consent to receive medical treatment which may be deemed advisable in the event of unjury, accident, and/or illness during this activity.I hereby consent NOT to hold directors, officers, employees, volunteers, representatives, and  agents responsible for damaged or stolen art during this activity.I understand while participating in this activity, I may be photographed.  I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns. 

I HEREBY  MAINTAIN THE RIGHT to withhold photographing of my art without expressed permission by the Midland Palette Club and their directors, officers, employees, volunteers, representatives, and agents. The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.

BY SUBMITTING I HAVE CERTIFIED THAT I HAVE READ THIS DOCUMENT AND I FULL UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
