    Fall Membership Retreat Registration Form

  • PLEASE READ THESE INSTRUCTIONS before completing this form! 

    1. You may register yourself and up to 12 additional people with the registration form. If you want to register more people from your club than 13 total attendees please complete an additional registration.

    2. All attendees will be housed in cluster cabins on double bunks, therefore listing preferred roommates is not required. Sheets, pillows, blankets, towels, and toiletries are not provided.

    3. The registration fee for this event is $110.00. This fee includes lodging, four meals, meeting space, conference activities, workshop materials, conference t-shirt, and recreational activities.

    4. The registration deadline is Sunday, October 22, 2023. A completed registration packet will include the registration and full payment. Full payment must be made by the registration deadline or you will be subject to a $30 late fee and are not guaranteed a conference T-shirt. Registrations placed after October 22, 2023 are not guaranteed.  Contact District Administrator Virginia Braswell to check for availability. You must check for availability prior to arrival, if registration was late. You must also call District Administrator Virginia Braswell to make a substitution if a registered member cancels.

    5. Method of payment: We will accept personal/school checks, credit cards via online registration, credit cards via the authorization form, and money orders (please do not send cash through the mail).  If you need a credit card authorization form please contact District Administrator Virginia Braswell.  If registration fee is not paid in full and postmarked by October 22, 2023, you will be subject to a $30 late fee that will be due upon arrival at the event. 

    6. If submitting multiple registrations at one time you should select add member on the registration system and provide the information that is asked.  You may then either pay via a credit card or mail a form of payment.  Please include a cover note indicating the fees being paid for each registration enclosed.  Make checks payable to the Georgia District of Circle K Conferences and Conventions (GA CKI Con-Con). Mail payment with this completed form to FMR 2023, c/o Georgia CKI P.O. Box 6618 Macon, GA 31208.  All requests concerning tax and vendor forms or payment directly from a school must be made prior to the deadline by contacting District Administartor Virginia Braswell.

    7. Cancellation requests must be made in writing or you will not receive a refund. Cancellations made prior to October 22, 2023 will be refunded in full.  There will be a $60 deduction per registration for cancellations made between October 22 and October 25, 2023.  Cancellation requests postmarked on or after October 25, 2023 will not be honored.

    8. You will be sent medical form electronically to complete along with a newsletter for the weekend 7 days prior to FMR.
