Please ensure that you have gathered the following information before submitting this form:
- KLA Unit information for each of your classes/year levels/stages - This should be available from your scope and sequence, but please confirm with class teachers.
- Review your last semester report and submit any requests regarding design/layout changes required.
- Information about your comments and preference for use of comment banks or not.
The information about modified reports, attendance and teaching staff will be collected during Terms 2 and 4 as a separate submission.
To complete this form should take a maximum of 5-10 minutes if you have all the required information available.
