    • Policies & Procedures 
    • Scheduling

      It is important that we maintain a proper Instructor to Student ratio for your child’s preferred session(s) to get the most out of the Eye Level program. Please attend Eye Level only on your scheduled day(s) and time(s). If there is a need for a temporary schedule change or a planned absence, a 24 hour notice by emailing or calling to Eye Level Livingston  is required. In case, sufficient notice is not provided then we may not be able to accomodate your request. We request that any permanent schedule change be submitted, in writing, to Eye Level Livingston management. All schedule changes must be approved by Eye Level Livingston management.

    • Class Attendance

      Students are required to attend 1 (or 2 classes in some cases) per week, at their designated day and time. Walk-ins out of their schedule are not allowed and will not be entertained until unless schedule change was previously approved. Please make every effort to arrive on time. Tardiness interrupts the other students and will result in a shorter class for your child. Make-up times for late arrivals are not guaranteed. If your child is going to miss a class, please call ahead to arrange homework pick-up in lieu of a make-up class so that your child doesn’t miss a week of work. Please drop off any completed work at the same time.

    • Pick-Up

      • Please make every attempt to pick your child up at the time their session ends. Tardiness during pick up detracts teachers from the next students’ session.
      • When your child’s scheduled session has ended, all Eye Level work will end while they await pick-up.
      • The parent/guardian should come into the center to pick up their child. This is absolutely necessary for safety of your child.
      • It is the responsibility of the parent/guardian to ensure their child’s safety outside the child’s class hours. Eye Level Livingston does not provide child care, pick up or drop off services.
      • Please do not arrive to pick up your child while under the influence of drugs or alcohol.
      • Children will only be released to a custodial parent, guardian, or caregiver. Picture ID is required.
      • All parents or caregivers are responsible for signing your child out at the end of each session on the sign-out sheet at the front desk.
      • Please let us know if there is someone who should NOT be allowed to pick up your child.
      • It is your responsibility to notify Eye Level Livingston if someone else is picking up your child. We require picture IDs to be confirm who is picking your child up. You can call or send a note that is signed and dated stating your permission for someone who is not on our list to pick up your child.
      • Parents/caregivers are required to come into Eye Level Livingston to pick up your child. Parent must sign their child out with their signature and the time on the parent/guardians sign out form.
      • If your child is in the classroom when you come to pick him/her up, please do not interrupt the class. If you need to pick up your child immediately, have a staff member alert the teacher so your child can be dismissed.
      • A $10 late fee per child will be applied every 15 minutes that a parent or guardian is late.
    • Missed Sessions / Make-Up Policy

      •  A 24-hour notice is required for all planned absences. Please contact Eye Level via phone (973) 992-8008 (voicemail is acceptable) or e-mail (Livingston@MyEyeLevel.com) regarding your missed class. In case of an illness, or unavoidable circumstances, please contact Eye Level no less than 2-hours prior to your scheduled session. Sessions missed with proper notification will be regarded as ‘Excused’. However, no refund/credit will be given for missed sessions.
      • Excused Absences are eligible for make-up sessions. Make-up sessions do not expire, and must be scheduled and used within a month where full tuition has been received. Make-ups cannot be used to extend enrollment or prorate tuition.
      • Unexcused Absences and all other absences for which Eye Level was not notified and is not be eligible for make-up sessions.
      • In any case, if your child is going to be absent for a class, whether planned, excused or unexcused, please get in touch with us either by phone/e-mail/in-person at your earliest convenience to pick up homework in lieu of a make-up before the next scheduled session, so that your child doesn’t miss any work. Please drop off any completed work at that time.
      • If your child will be away for more than 2.5 months, a re-enrollment fee of $50 will be charged upon reinstatement.
    • Sick Policy

      • Please do not send your child to Eye Level if they are ill. If your child missed school due to an illness, they should also be considered too ill to attend Eye Level.
      • If student has a communicable disease, the student should be symptom free for 24 hours before attending classes at Eye Level Livingston. If the communicable disease is a common cold, then the student can attend class if student does not have a fever.
    • Program Withdrawal / Refund Policy

      • A 30-day written notice is required for withdrawals from regular enrollments. Any deposit paid will be applied to the last month. Failure to notify will result in a loss of deposit or charge of a service fee equal to the amount of booklets ordered for the student.
      • If Eye Level believes your child fails to adjust to the program, the privilege of attending will be withdrawn.
      • We reserve the right to dismiss anyone from our program for disrupting our program or not following the rules. Parents will be notified in writing and any unapplied funds will be returned, minus a 30% withdrawal fee (per month remaining in the agreement).
      • Parents who wish to withdraw their child for any reason must do so in writing and must speak with the director. Any unpaid balances must be paid at that time. You are responsible for the full unpaid balance of your contract. 30 days’ notice is required for all withdrawals from the program.
    • Tuition Payment & Deposit

      • Payment is due on the 1st of each month.  
      • A $2 per day late fee will be assessed for payments made after the 5th of the month in which tuition is due. Parents are responsible for any overdraft and fees due to bounced checks incurred by Eye Level.
      • Checks should be made payable to “Eye Level Livingston”.
      • If payment is delinquent by more than 2 weeks of the payment due date, Eye Level Livingston will suspend your child from the program. You will receive written warning if Eye Level Livingston suspends your child. All payments, late fees and any outstanding fees must be paid before the suspension can be lifted.
      • Enrollment of each student requires a registration fee of $50 and a non-refundable deposit equal to the one month’s tuition. The deposit will be applied to either the last month upon withdrawal of the student, or applied to a month when dues were not received by the due date.
    • Booklet Policy

      The costs of booklets assigned during the session are included in the tuition fee. Additional booklets are charged at $5 each. Students will be provided booklets to do at home. Please bring back all the work assigned. We will then grade it and work on corrections, if any with the students. After the corrections, the books are handed over to the respective student to keep at home. Note that all homework is optional but the program benefits the students when homework is completed at the same pace as it is assigned to them. If the student does not complete assigned work, they will do it during the next session which would slow down student's progress in the program.

    • Tuition Freezes / Notice of Extended Absence

      An extended absence is defined as a student missing more than 4 consecutive classes. Notice of the student’s extended absence must be received in writing, no less than 30 days prior to the first date missed. During an extended absence, the student can either receive booklets in lieu of make-ups, or freeze tuition for up to 75 calendar days. Any re-enrollment after 75 calendar days will result in a re-registration fee of $50. The student’s existing timeslot will only be held for 1 month. Timeslots can be permanently held upon payment of tuition for that month. If the absence extends past 4 weeks of unattended classes, re-enrollment into the same slot cannot be guaranteed.

    • Referral Credit

      Parents who refer the Eye Level program to another family will receive a $50 tuition credit per new family enrolled. Parents of the new student enrolled will ALSO receive a credit of $50. Please note: Tuition Credit will accrue at the end of 3 continued months of enrollment of the referred family and will automatically reflect in your subsequent invoice.

    • Center Holidays / Closings

      The center will be closed during some public holidays. Please refer to our yearly calendar for the dates when center is closed. Make-ups do not expire, but must be used within a month where full tuition has been received. Make-ups cannot be used to extend enrollment or to prorate tuition. Eye Level reserves the right to cancel any scheduled session. Every effort will be made to communicate this timely to the parents of enrolled students.

    • Emergency Situations

      • Fire extinguisher is located in the back area.
      • In the event of a fire, staff and students will be relocated as directed by the fire department professionals.
      • Any fire, large or small, the staff or the director will report it to the fire department at once.
      • Parents will also be notified as soon as possible.
      • Instructors/mentors will always remain with the children.
      • In case of severe illness or injury, staff will render first aid to the best of their knowledge and ability. Another staff member will call 911, if needed, and move other children to another room if possible. Parents will be notified after 911 is called.
      • If a child must be taken to the hospital, staff over the age of 18 will go with the child until parents can arrive.
      • Parents must provide an epipen, inhaler, or other emergency medicine if their child requires it in the event it is needed. In this case, center director must be informed of the condition and if any one other than child will have to assist the child in administering the needed medicine.
    • In-Center Rules

      • It is expected that all children will behave in a suitable manner and show respect, care for safety, and follow directions of the instructors.
      • If discipline issues continue, parents will be called to have a conference with the director.
      • Please limit food during class. Small snacks are permitted, but must be nut-free.
      • No yelling or running is allowed.
      • No playing is allowed in the corridors.
      • Parents are not permitted to enter the classrooms as to reduce distractions for the students.
      • Unruly students will be dismissed early from class for the day, but required to continue to do their assignments at home.
      • No smoking, tobacco, or alcohol is allowed in building
      • Parents are encouraged to remain in the center while their children are in class. If parents need to step away, provide a contact number that can easily reached.
    • Cell Phone/Electronics Usage

      • Children in the Eye Level Livingston Center are not allowed to use cellular phones, smart watches for phone calls,
        or other electronics.
      • If needed, your child may use the business phone to call a parent
        or guardian.
      • If you need to speak to your child, please call the main number. We do not encourage the use of phones while the
        children are here.
      • We do not allow children to play with electronics while at Eye Level Livingston. 
    • Change of Address/Contact Information

      • Please notify Eye Level Livingston Center of any change in address or telephone number (home, business, or cell) as soon as it is effective.
      • Current and up-to-date information is of utmost importance in case of emergency or a child’s illness.
    • Child Abuse Prevention

      Eye Level Livingston Center must report any suspected child abuse to the Essex County office of the NJ State Child Protection and Permanency
      (formerly DYFS).

    • Lost & Found

      • Found items will be in the Lost and Found box in the front.
      • Please mark your child’s belongings with their names or initials.
      • Eye Level Livingston is not responsible for lost or stolen items.
    • Medication

      Eye Level Livingston staff is not trained to administer medication. If any medication is required for a student, it should be administered by the parent/guardian before/after the class under their supervision.

      Any emergency medicine such as but not limited to epipen, inhalers if are or may be required by the student, should be pre approved by the center director. This is to ensure the center director can communicate clearly with the parent, child, and the instructor on how to proceed in case of emergency that requires administration of the emergency medicine.

    • Parental Notification

      • If at any given time a situation arises that requires Eye Level Livingston to notify parents, i.e. medical emergency, communicable
        diseases, or severe weather conditions, we will notify parents via email.
      • Parents will be notified in person or by phone of any concerns or questions Eye Level Livingston has about a student.
      • Parents will be notified via email concerning any changes in policy or procedure.
    • Photography

      We take pictures of the students during event/competition participation for our Facebook and Instagram pages and our website. These are the property of Eye Level Livingston.

       

    • Video and Audio Monitoring

      Our tutoring center utilizes Ring or similar cameras for security and monitoring purposes. These devices may capture short clips with both video and audio recordings. By enrolling in the program or entering our premises, you acknowledge and consent to the following:

      • Purpose of Monitoring:Cameras are installed for the safety and security of our students, staff, and property. They also assist in monitoring educational activities and ensuring quality control.
      • Areas of Coverage: Cameras are placed in [specify locations, e.g., "all common areas, classrooms, and entrances"]. There are no cameras in private areas such as restrooms.
      • Recording and Storage: Both video and audio clips may be recorded and stored securely for a period of [specify duration, e.g., "30 days"], after which they are automatically deleted unless needed for a specific incident investigation or legal purpose.
      • Access to Recordings: Access to recorded material is strictly limited to authorized personnel for legitimate business purposes, such as investigating incidents or improving our services.
      • Privacy Protection: We are committed to protecting your privacy. Recordings will not be shared with third parties unless required by law or with explicit consent from affected individuals.
      • Prohibited Actions: Attempting to tamper with, obstruct, or disable the cameras is strictly prohibited and may result in disciplinary action.

      By continuing to use our services or entering our premises, you indicate your understanding and acceptance of this policy.

    • Non-Solicitation of Staff

      The client agrees not to solicit, hire, or employ any current or former staff of the Center for private tutoring or related services during the term of this agreement and for a period of 6 months following the termination of this agreement. Any violation of this clause may result in the immediate termination of services and other legal remedies as deemed appropriate by the Center.

    • Liability

      Eye Level Livingston will not responsible for any injuries that may occur while at the center. Parents/guardians are responsible for their child’s safety outside of the child’s class hours. If a student destroys the property of the center or causes harm to another student, then the parents will be held responsible and will need to pay for the damages and take responsibility of their child's actions.

    • Meeting with the Center Director

      • The Center Director will be your primary contact point at the Eye Level Center to inquire about your child’s progress.
      • The Center Director may not be available for individual conferences during center open hours. If you would like to inquire about your child’s progress or communicate directly with the Center Director, please utilize the key and note or book a slot for meeting with center director using this google calendar link.
    • Receipts and Statements

      • Receipts for the monthly Eye Level program will only be supplied if requested by a parent. We will only list the monthly tuition charge.
      • A statement may be requested at any time; however, we require one week’s notice to prepare the invoice. We will email a PDF of the statement, but if you require a hard copy, you must pick it up in person. We do not mail invoices.
    • I have read and agree to the terms and conditions of this agreement, and I further agree and understand that my child(ren) can and will be removed from the program if the aforementioned policies and procedures are not complied with. I further agree to hold Eye Level Livingston and/or any of its nominees, harmless from and against any and all liability as to any problems or injuries that could arise. Regardless of fault, Eye Level Livingston is not responsible for loss of, or damage to, any property left behind by my child. I hereby assume all risk of such loss or damage and agree to defend, indemnify, and hold Eye Level Livingston harmless from all claims. I understand and agree that I will pay for all costs and/or expenses of any kind for damages that may be caused by my child.
