I will commit to a private, One on One meeting with a Camp Mult Trainer to meet the dog to see if the dog is a good fit(Yes or No)..

I will keep my dog on a regular routine of heartworm preventative(Yes or No).

I will agree to pay the adoption fee that Camp Mutt posted for the dog(Yes or No).

I am prepared to make a 10 to 15 year commitment to this dog. The dog will have appropriate food, water, shelter, and medical care for the duration of its life. (Yes or No)

I will not give this dog away, take it to a shelter, place him/her with another person or family member, or sell this dog to any other person. You must return dog to Camp Mull if you no longer want the dog. No refunds.(Yes or No)

I am financially able to provide routine and emergency care for this dog for his/her lifetime. This includes but is not limited to food, boarding (if necessary). regular vet care. heartworm preventative and flea and tick preventative.(Yes or No)

I will provide flea/tick control as needed. I will work with my vet and agree on a regular schedule for wellness visits, vaccinations, and any other tests we agree are necessary for the health and well-being of my dog. I will not chain or tie-up my dog and leave it outside alone. I will keep & care for this dog in a safe humane manner. My dog will be an indoor dog and an important member of my family. I agree to abide by all state and local animal control and leash laws. I understand it is my responsibility to become familiar with these laws and to license the dog according to all regulations. I understand that Camp Mutt makes no guarantees or warranties regarding the health or temperament of this dog. I agree to be fully and solely responsible for this animal, and for any damages that may result from its actions. Camp Muff shall not be held liable for the behavior of this dog or any damages it may cause. Iwill not give this dog away, take it to a shelter, place him/her with another person or family member, or sell this dog to any other person. You must return dog to Camp Mull if you no longer want the dog. No refunds.

This dog has been seen by a licensed Veterinarian and is up to date on all shots.

I enter into this contract of my own free will and understand that this is a binding contract enforceable by civil. Once you meet the dog and we agree it is a good fit. You will be approved to adopt.
