COMPETITOR DETAILS

PLEASE NOTE: 

1) All divisions are included with registration fee. To help lessen the expenses of the tournament (less expense=more donated to St. Jude) the tournament director will combine age groups and/or divisions. This will depend on the number of competitors and medals available. 

So, there may be a case where you register for both CMX Forms AND Traditional Forms but they will be combined at the tournament into one division. You will simply pick which one you want to do at the event. 

A final division breakdown will be posted on the website at the 1st of December.

2) "Little Ninjas Challenge" is for ages 6 and under for students that do not participate in form/weapon/sparring. Competitors cannot participate in both Little Ninjas Challenge and "Regular Divisions".

I, (“Participant”) desires to participate in the KICK-A-THON TOURNAMENT event, I understand that participating in a KICK-A-THON TOURNAMENT event is contingent upon my review, understanding, and acceptance of this Release and Waiver.

Participant HEREBY AGREES TO WAIVE, RELEASE, DISCHARGE, AND HOLD HARMLESS ST JUDE KICK-A-THON and their respective members, managers, officers, directors, owners, employees, pastors, deacons, volunteers, agents, contractors, and representatives (collectively “Releasees”) from any and all actions, demands, suits, complaints or other claims that I, my assignees, heirs, distributes, guardians, next of kin, spouse and legal representatives now have, or may have in the future, for all liability for any loss or damage whatsoever, including injury, death, or property damage (collectively “damages”) related to the Participant’s participation in the event or any negligence or other acts, whether directly connected to the use or not, and however caused, by any of the Releasees, while the Participant is in, upon or about the event and/or its surrounding premises.

Participant also waives, releases, and discharges the Releasees from all liability for any loss or damage, including personal injury, death, property damage, medical expense and any other type of expense related to the condition and/or maintenance of the venue, any equipment, or the premises where the use occurs, whether or not I am then participating in the use of the venue. Participant also agrees to release, hold harmless, defend and indemnify the Releasees from any and all liability for any loss, injury and/or damages to any third party arising out of participation in the event by the third party.

At all times during the event and/or its surrounding premises, Participant agrees to follow, abide by, and adhere to all published guidelines, orders, and recommendations related to the coronavirus disease, also known as COVID-19 and SARS-CoV-2, from the Centers for Disease Control and Prevention (CDC),  or similar governmental body or agency.

This Agreement shall be binding on all successors in interest, transferees, or beneficiaries of the parties. It shall not be modified except by a written agreement initial by both parties. In the event that one provision of this agreement is found to be invalid or unenforceable, the remainder of the Agreement shall be severed and enforceable as written. The failure of a party to enforce any provision in the agreement shall not act as a waiver to its ability to enforce the agreement thereafter. This written agreement represents the entire agreement between the parties on the subject matter contained herein. Any prior agreements, promises, representations, or warranties related to the subject matter of this Agreement are hereby void and superseded by this Agreement.

I HAVE READ THE FOREGOING TERMS AND CONDITIONS AND AGREE TO BE BOUND BY THIS AGREEMENT.
