This form is to be completed dated, and signed for all minor students attending my vocal and or guitar lessons. A minor is a student under the age of 18 at the time of lessons.
I hereby release ELISA SELAH RIVERA-VAZQUEZ for any and all claims for damages, injuries, demands, or causes of action I, or my family members, my heirs, executors, administrators, or assigns, may have or assert arising out of attendance by the minor named below, or the participation in lessons/activity for students belonging to ELISA SELAH RIVERA-VAZQUEZ.
