Through meal planning, grocery shopping, cooking techniques, and nutrition education, participants will gain the confidence and knowledge to create delicious, nutritious meals. The program will also day excursions for a real-life farm-to-table experience.

    Location: 9B/4-8 Burke Crescent, Northlakes
    Day and Time:   Every Wednesday, 10:00 AM - 3:00 PM
    Cost:   $2700 for 10 weeks - $270 per week.
    Class size: 6 students per class.

  • Service Agreement Terms and Conditions: Cooking Program

    This Service Agreement is made for the purpose of providing products and services under Participant’s NDIS plan. The parties agree that this Service Agreement is made in the context of the NDIS, which is a scheme that aims to:

    1.    support the independence and social and economic participation of people with disability, and

    2.    enable people with a disability to exercise choice and control in the pursuit of their goals and the planning and delivery of their supports.

    This Service Agreement ("Agreement") outlines the terms and conditions governing your participation in the cooking program provided by Supreme Community Care, hereafter referred to as the "Provider." By enrolling in the Program, agree to adhere to the following terms and conditions:

    Program Duration and Cost:

    The Program runs for a duration of 10 weeks.

    The total cost for the 10-week Program is $2700.

    The cost is allocated at a rate of $270 per week for 10 weeks.

    Invoices for the Program's weekly cost will be submitted to the appropriate funding manager.

    Cancellation and Attendance Rules:

    1.     Short notice cancellation rules do not apply to this specific program arrangement.

    2.    Supports within the Program can be claimed against the Participant’s plan regardless of attendance.

    3.    If a Participant stops attending the Program without providing notice, the Provider may only continue claiming fees for a maximum of four (4) consecutive weeks from the time attendance ceased. This situation is deemed an "unplanned exit."

    Notice Period for Exiting the Program:

    1. To exit the Program of support, the Participant is required to provide a minimum notice of two (2) weeks.

    During the 2-week notice period, 100% of the scheduled fee will be charged for any sessions taking place.

    General Terms:

    1. This Agreement constitutes the entire understanding between the Participant and the Provider regarding the Program and supersedes any prior agreements or understandings, whether written or oral.

    Any modifications to this Agreement must be made in writing and signed by both parties.

    The Provider reserves the right to make reasonable changes to the Program schedule, content, or other aspects as deemed necessary. Participants will be informed in a timely manner.

    The Participant is responsible for ensuring timely communication, attendance, and adherence to the terms of this Agreement.

    By enrolling in the cooking program, you acknowledge that you have read, understood, and agreed to the terms and conditions stated in this Agreement. Failure to comply with these terms may result in consequences outlined within this Agreement.
