  • Please fill out permission for each enrolled child in your family.

  • Walking Field Trips

  • Permission to participate in walking field trips off school grounds, where other transportation is unnecessary.

  • Bussed Field Trips

    (Applicable once children reach the age of four).
  • Permission to attend field trips with Laker Prep Preschool on Becker County Transit buses. Laker Prep Preschool will notify families of field trip prior to attending.

  • Internet

    (All internet activity will be closely monitored, and sites will be selected by Laker Prep Preschool and Early Childhood Center staff).
  • Permission to use the internet to access educational websites that enhance learning.

  • Photos with Names

    (Pictures with names used of the children for classroom purposes to enhance learning and celebrating such as artwork, bulletin boards, cubbies, attendance, school jobs, school yearbook).
  • May have his/her picture taken and label their name at Laker Prep Preschool and Early Childhood Center to celebrate and enhance learning activities within the school.

  • Pictures in Advertisements

    (Families will also be notified prior to any pictures being released for advertisements).
  • Permission to use his/her picture in an advertisement for Laker Prep Preschool and Early Childhood Center.

  • School Website

  • Permissions to Laker Prep Preschool or Early Childhood Center permission to post classroom and field trip pictures on the secure website, shutterfly.com. The schools will only invite the immediate families of Laker Prep to this website, and they will then register themselves. No pictures will be available to the general public, nor will they be tagged.

  • Social Media

  • I understand to only post pictures of my own student(s) on social media while attending a school event at Laker Prep Preschool or Early Childhood Center.

  • Topical Ointments

    (Laker Prep Preschool will apply sunscreen, insect repellant, over the counter anti itch cream or hand sanitizer on students when appropriate).
  • Permission to have topical ointments applied in appropriate situations.

  • Research

    (In the event Laker Prep Preschool or Early Childhood Center is asked to be involved in a research study, experimental procedure, or public relations event, parents will be asked for further permission).
  • I will consider participation in the event Laker Prep Preschool or Early Childhood Center is asked to be involved in an above activity, if notified and informed of activity.
