As a participant in this workshop, I acknowledge the following and consent to participate:
I understand that Gestalt therapy works at a deep emotional level and that some personal information about me may be shared. At all times I have the right to decide on the degree of my participation, and to ask for further explanation or support. Sessions will include experiential interventions and role-play as appropriate. In the group environment, I may be asked to consider my beliefs, feelings, perceptions and behaviours resulting in insights which may be outside of my comfort zone.
I agree to the following:
1. I will hold strict confidentiality regarding all matters (names, events, etc.) pertaining to the personal therapeutic work or identity of other participants in the group. I will not disclose information about another group member outside of my group verbally, or through any written or electronic means including email, Facebook, twitter and any other public forums. This contributes to creating a safe place for participants to share.
2. All information shared during the group is confidential. However, the facilitator(s) is/are required to disclose confidential information if any of the following conditions exist:
a) If you tell the facilitator(s) that there are reasonable grounds to suspect that a child is need of protection due to physical harm, neglect or sexual abuse by a person having charge of the child, the facilitator(s) have to report this to the Children’s Aid Society.
b) If you disclose that another health care provider has sexually abused you, the facilitator(s) have to report it to the appropriate college.
c) If you tell the facilitator(s) that you’re planning to cause significant bodily harm to yourself or someone else, they have to intervene to make sure that you or the other individual is safe.
d) If you are involved in a court case and the records of the facilitator(s) are subpoenaed, or if our office is presented with a search warrant, the facilitator(s) will do their best to keep your records confidential, but sometimes it may be out of their control to do so.
ONLINE WORKSHOPS
Currently we are offering our workshops online during the pandemic. Benefits of online workshops include the ability to offer access where learning would otherwise be unaccessible and the continued opportunity for experiential learning in groups. Reasonable means will be used to protect security and privacy of communications over the online platform including meeting ID for participants only, password protection, entry only through waiting room feature controlled by GIT instructor, screen sharing by the host only and no recording of sessions without participant agreement. However I understand that there are risks and consequences including but not limited to the interruptions, technical problems and potential for unauthorized access so that privacy and security cannot be guaranteed.
If you have any questions about the potential impact of therapy or limits of confidentiality, please discuss them with your instructor.
