In order for the nonprofit organization Victims of Communism – Florida, Inc., to ensure that students who have registered on our website to compete in our scholarship program have their parents’ or legal guardians’ permission to do so, we require this form to be completed and electronically signed by a parent or legal guardian. This form will be stored in a secure location on the website.
