Forest Voices Children's Choir
In the year 2020, the World Health Organization declared the novel Coronavirus (COVID-19) a worldwide pandemic. Today, three years later, COVID - 19 is no longer classified as a pandemic, but it is still present in our community. Due to the capacity of COVID-19 and other illness to transmit from person-to-person through respiratory droplets, singing can increase the likelihood of the spread of such illnesses. This waiver addresses the possible risks of COVID - 19 transmission through participating in Forest Voices Choir, as well as other possible risks.

It also addresses the taking of photographs or video, which may occur for promotional purposes with this group.

By signing this waiver, you acknowledge that you and your children participate in Forest Voices Choir at your own risk and that you are willing for yours or your child's image to be used in promotion of the group.
In consideration of my child's participation in the foregoing, the undersigned acknowledge and agree to the following:

I am aware of the existence of the risk on my child's and my physical appearance to the venue and my child's and my participation in the activity of Forest Voices Choir that may cause injury or illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.I take responsibility for doing my part to mitigate the spread of COVID-19 or other illnesses at Forest Voices Choir by keeping my child home when they feel sick, when there is a known case of COVID in our immediate family, or in the case of any other known prolonged exposure where they were indoors, unmasked, and in close contact another person who had COVID-19 at the time of their meeting - (for example, an indoor, unmasked playdate with a friend who had COVID-19.)In the event that I or my child experiences possible symptoms of COVID 19, regardless of whether we have a known exposure, I take responsibility for testing and, in the event of a positive COVID test, with or without symptoms, keeping my child home from Forest Voices Choir until we get a negative test.I am aware that singing increases the likelihood of person to person spread of COVID and other airborne illnesses, and that singing without a mask further increases the risk of possible transmission. I am aware that masks are optional at Forest Voices Choir, and that some may choose to sing without wearing masks.Beyond just COVID, I acknowledge that participating in Forest Vocies Choir carries with it the potential for injury, including serious injury, for other reasons. The risks include, but are not limited to: actions of other people including, but not limited to, Noe Venable, participants, the venue, weather, and / or other natural conditions. With full awareness of these risks, I support my child's participating in Forest Voices Choir.
Following the pronouncements above I hereby declare the following:

I, the undersigned, am the parent and/or legal guardian of the minor child listed on the first page of this form. I hereby fully release, waive, foreverdischarge, hold harmless and agree not to sue Noe Venable or the venue, and any possible affiliates from any and all liability arising out of or in connection with my child’s participation in Forest Voices Choir, and all liabilities associated with any andall claims related to such participation that may be filed on behalf or for my child. For the purpose of this release and waiver, “liability” means all claims,demands, losses, causes of action, suits or judgments of any and every kind that arise as a result of my child’s participation in the Forest Voices Choir and that resultfrom any cause other than the Parties’ gross negligence.By signing below I give permission for my child to participate in Forest Voices Choir. I also give my consent to any medical treatment deemed necessary by medical personnel for the physical well-being of my child. I assume full responsibility for my child’s behavior and agree to pay for all damages to property or person caused by him/her/them. I understand that I will be notified if my child’s behavior is disruptive to an extent that compromises the experience for other participants, and that further behavioral issues may result in his/her/their being removed from the program. This release and waiver shall remain in effect while my child is participating in the program. Iunderstand that I may revoke my consent in writing. However, if I do so my child will no longer be permitted to participate in the program.I understand that this release and waiver is intended to be as broad and inclusive as permitted by the laws of the State of California, and agree that if any portion is held invalid, the remainder of this release and waiver will continue in full force and effect. I acknowledge that I have had sufficient time to read this entire form. I have carefully read and understand all of it and I agree to be bound by its terms.
Media Release Statement:

This choir includes a final concert which will be captured on video and in photos. I understand that by participating in Forest Voices Choir, my child may appear in said documentation, which may be used for future promotion of this group. In this context, I agree to let my child's photo, video, or film likeness to be used for any legitimate promotional purpose by Noe Venable as director of Forest Voices Children's Choir.
By signing below I acknowledge that I have read the foregoing Liability and Media Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing it as my own free act and deed; that I give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
