Please note that this reservation does not require a deposit at this time.

An online "Quickbooks'' invoice will be issued to you once this form is submitted. Your booking is confirmed when payment is received. Appointments must be confirmed 48 hours prior to arrival.

If you cancel your event after confirmation a refund will be issued via EMT, less $50.00 for administrative fees.

Please be aware that the QI Clay Club is a micro business (of one person) and in the event that the QI Clay Club is unable to host the event (due to illness, injury, etc.) you will be offered the option to reschedule or a full refund will be issued.
