By completing this form I am submitting an appointment request and price quote inquiry. In response I will receive an text message with the price for my service and may be adjusted upon further consultation. I will also receive an appointment date and time options that coordinate with my times listed above. Generally new guests can be seen within 1-2 weeks of an appointent inquiry, guests with limited availability may have to wait up to 3-4 weeks to be seen.
I understand that once a date is selected I will receive an invoice for a $25 deposit to hold the spot for my appointment. This $25 covers our consultation for the service and is non refundable. I understand that this deposit will be applied to my appointment and taken off the service cost the day of my appointment. I understand that the date of my appointment will not be held for me until this appointment deposit is received.
I understand Esthetician Jana has strict 24 hour cancelation policy and if i do cancel my appointment with less than 24 hours notice, or no show my appointment, I may be subject to a late cancelation fee of up to 100% of my booked service.
