Dance Policy Form

ADMISSION FEES - NON-REFUNDABLE COST is $40.00 for non-SJBH students, and $35.00 for SJBH Students. All registrants must submit a copy of either current verification of enrollment in a high school program or a copy of their family's Declaration of Intent for the current year. SJBH students do not have to provide proof of enrollment with their registration form.

If registrant is homeschooled, please attach a Declaration of Intent for the current 2022/23 school year. If registrant is attending a high school program, please provide verification of enrollment for the current year. Picture of School ID Card is acceptable as a verification of enrollment. SJBH students may skip this step.

ACKNOWLEDGEMENT

By submission of this form, Parent/Guardian/Student acknowledges the truth of all statements on this registration and acknowledges the risk of COVID 19 associated with any gathering. Parent/Guardian/Student also agrees to take full responsibility for his/her child and hereby agrees to hold St. John the Baptist Hybrid and the affiliates and subsidiaries, the officers, directors, agents, and partners and all individuals associated with the organization, promotion and establishment of this event harmless against all claims, losses or damages to persons or property, charges or fines and attorney's fees arising out of or caused by your child's actions or inaction during occupancy or use of designated premises or a part thereof. Parent/Guardian acknowledges that pictures and video of my child taken during the event may be used for future event promotions.
