Participation and Supervision: I acknowledge that for the "Creative Kids Corner" toddler program, it is required that a parent or guardian stays present for all program sessions, as specified in the program details. For the "Active Minds and Creative Spirits" senior program, I understand that my participation will be voluntary, and I will engage in the activities provided.

Photography and Image Release: I grant permission for the 29th Street Community Center to photograph and/or record my child or myself during program activities. I understand that these photographs and recordings may be used for program-related purposes, including but not limited to promotional materials, social media, and the organization's website. Any personal information shared will be handled in accordance with our privacy policy.

Liability Release: I acknowledge that I, or my child, will participate in program activities at my/their own risk. I release the 29th Street Community Center, its staff, and volunteers from any liability for injuries or accidents that may occur during program sessions.

Emergency Medical Treatment: In the event of an emergency, I authorize the 29th Street Community Center staff to seek medical treatment for my child or myself, including the administration of first aid or transportation to a medical facility, if deemed necessary.
