Elevate the Stage Athlete Gift Order Form

Thank you for competing at Elevate the Stage 2024! This year's athlete gift is a Custom GK tank leotard for all competitors. Leotards will be drop shipped directly to the athlete's gym. Every gym should place an order for each Elevate the Stage location that they're competing at. Only one form per gym per location should be submitted.
Toledo participants must submit their order forms by December 1, 2023.
