By signing below, you are agreeing to the following terms:
- All information provided within this document is accurate and true.
- If any provided information is found false and/or inaccurate, it may result in serious injury and/or death.
- Should any of the provided information be found to be intentionally provided in a false and/or neglectful manner the registrant's membership will be terminated immediately and will not be eligible for renewal.
- I understand that all fees associated with USA Boxing and Georgia Amateur Boxing Association (GABA) are the responsibility of the registrant and/or that of their parent/legal guardian and NOT the responsibility of Vercetti Fight Club.
- I understand that should I choose to compete in a sanctioned USA Boxing event, all registration fees are the responsibility of the registrant and NOT that of Vercetti Fight Club.
- I agree to provide all the required gear (wraps, boxing gloves, etc as necessary for the training and/or competition. I agree to return any equipment lent by Vercetti Fight Club and acknowledge that all equipment will remain their property during use.
- I understand that any/all equipment used during training or during open gym will be used strictly for its intended purpose(s) and if used for any other purpose could result in serious injury and/or death.
- I agree to ask an employee for proper instruction prior to using any unfamiliar equipment and understand that utilizing equipment without proper instruction will be at my own risk.
