Acknowledgement

I have read my copy of the Employee Handbook. The employee handbook describes important information about IMMERSE Modern Massage, and I understand that I should consult the Management regarding any questions not answered in the handbook. I have entered into my employment relationship with IMMERSE Modern Massage voluntarily and acknowledge that there is no specified length of employment. Accordingly, either I or IMMERSE Modern Massage can terminate the relationship at will, with or without cause, at any time, so long as there is no violation of applicable federal or state law.

The Employee Handbook (Found here) and the policies and procedures contained therein supersede any and all prior practices, oral or written representations, or statements regarding the terms and conditions of your employment with IMMERSE Modern Massage. By distributing this handbook, the Company expressly revokes any and all previous policies and procedures which are inconsistent with those contained therein.

I understand that, except for employment-at-will status, any and all policies and practices may be changed at any time by IMMERSE Modern Massage, and the company reserves the right to change my hours, wages and working conditions at any time and I understand that any information that you agree to may be revised on this form or in the handbook and that may supersede, modify, or eliminate existing policies.

I understand and agree that nothing in the Employee Handbook creates, or is intended to create, a promise or representation of continued employment and that employment at IMMERSE Modern Massage is employment-at-will, which may be terminated at the will of either IMMERSE Modern Massage. Furthermore, I acknowledge that the handbook is neither a contract of employment nor a legal document. I understand and agree that employment and compensation may be terminated with or without cause and with or without notice at any time by IMMERSE Modern Massage or myself.

I have received the handbook, and I understand that it is my responsibility to read and comply with the policies contained in this handbook and any revisions made to it.

COVID-19 Policies

Per OHA and the 2020 COVID-19 pandemic, all employees must take notice of the latest rules and regulations for Personal Care facilities. By signing, dating and returning this document, you are acknowledging that::

I agree to be tested every 14 days if I start to have any of the symptoms (listed here: https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html) for COVID-19 and tested when knowingly exposed to the virus. I agree to provide proof of testing and acknowledge that failure to meet testing requirements will result in temporary removal from the studio and client schedule until completed. IMMERSE Modern Massage is not financially responsible for providing testing. Employees may access free testing sites​.

To help prevent the spread of COVID-19, I agree to:

Wash my hands often with soap and water for at least 20 seconds, especially before eating
Avoid touching my eyes, nose, and mouth with unwashed hands.

Avoid close contact with people and not shake hands.

Wear a face mask or cloth face covering that covers my nose and mouth when in public and/or when 6 feet of social distancing cannot be maintained.

Cover my cough and sneezes with a tissue and discard it in a closed container.

Clean frequently touched surfaces and objects.

Stay home if I feel sick and notify management immediately so they may handle my client schedule.

Stay home for at least 24 hours if I have a fever and after my fever is gone without the use of fever-reducing medicines, such as acetaminophen.

Keep sick household members away from others

Use soap and water, a bleach and water solution, or EPA-approved household products. *You can make your own cleanser with a mixture of 1 cup of liquid unscented chlorine bleach in 5 gallons of water.

Avoid sharing personal items.

I agree to ensure 6 ft. of distance between myself and colleagues/clients, except during the actual massage session.

I agree to ensure massage table sheets are changed between clients and massage tables are properly disinfected in between each use.

I agree to perform massages on customers in supine positions only when I’m equipped with a face covering. I agree to ensure that clients do wear face coverings when switching from prone to side or supine position.

Anyone at high risk for complications should talk to their healthcare provider for more information.

I agree to adhere to hygiene, cleaning, and disinfection requirements from the Centers for Disease Control and Prevention (CDC) and Department of Health (DOH) and maintain logs in the studio that document date, time, and scope of cleaning and disinfection.

I agree to conduct regular cleaning and disinfection at least after every shift, daily, or more frequently as needed, and more frequent cleaning /disinfection of shared objects and high risk areas. Ensure that workstations (e.g. chairs, headrests, work surfaces, massage tables, oil bottles, door handles) and reusable tools are cleaned and disinfected between each customer.

Client and Employee Policies, Booking Terms and Conditions

If Any of The Below Apply To You, Consult Your Physician Prior to Infrared Heat Use:

Individuals suffering from obesity or with a medical history of heart disease, low or high blood pressure, circulatory problems or diabetes should consult a physician prior to use. Individuals who are pregnant, have cardiac arrhythmia all medical conditions or those on medication should consult a physician and or licensed health care provider.

-Chronic Conditions / Diseases Associated with Reduced Ability to Sweat or Perspire
-Hemophiliacs / Individuals Prone to Bleeding
-Fever & Insensitivity to Heat
-Pregnancy
-Joint Injury– Recent (acute) joint injury should not be heated for the first 48 hours or until the swollen symptoms subside. Please let your practitioner know.
-Implants– Metal pins, rods, artificial joints, or any other surgical implants generally reflect infrared waves and thus are not heated by this system. Nevertheless, you should consult your physician
prior to using.
-Surgeries
-Pacemakers / Defibrillators
-In the rare event that you experience pain or discomfort, immediately notify your practitioner and to discontinue use.

Cancellation Policy

We’ve reserved a time just for you, and our therapist will be on-site ready to take your appointment when you arrive.

Immerse Modern Massage has a 24 hour cancellation policy. If you need to change or cancel your appointment, you must notify us at least 24 hours in advance.

Clients who no-show or late cancel (within 24 hours of their appointment) will be charged in full. Last minute cancellations cannot be refunded or used as credit. Discounts and promotional offers cannot be applied to the cancellation fee.

Please note that arriving late to your session may result in a shorter appointment so as to not penalize the clients booking after you. Clients who are over 20 minutes late will be asked to reschedule their appointment, and will be charged a cancellation fee. We charge in full for same day cancellations and no-shows, unless in cases of illness or emergencies.

Refund Policy

Due to the nature of our business, IMMERSE Modern Massage does not offer refunds for massage services, gift cards or packages/plans of care. Gift cards and store credit are non-transferable and cannot be shared. Packages are nonrefundable.

Consent To Treatment:

I understand there are benefits and risks for massage therapy and give my consent for treatment. I understand that there shall be absolutely no liability on the practitioner, IMMERSE Modern Massage and release them from any possible liability, damage or loss that may occur due to massage, stretch, exercise or lifestyle recommendations, use of CBD, essential oils, infrared heat or sound infused massage table, tripping, falling, personal injury, potential risk of COVID-19, or for any other reason whatsoever. As professional licensed therapists, we’re dedicated to providing a completely safe and enjoyable experience for our clients. Our work is strictly therapeutic and non-sexual. Any sexual comments or advances will result in immediate termination of the session. Recommendations made by our massage therapists or in videos on our YouTube channel or sent by any form of digital media of communication are at no point to considered to be a “prescription” which can be only given by a qualified doctor, physician or other medical professional. If you are concerned about your health in any way, please seek the help of a qualified physician before undergoing any of the treatments listed above. I have informed the massage therapist of all of my known physical conditions, injuries, medical conditions, any medications, drugs, alcohol, supplements, herbs etc, I am taking in this form. It is also understand that the massage therapist reserves the right to refuse treatment to anyone.
