  • After receiving this application Midtown staff will email you information regarding the process for completing your Background check and Health Screening.  

     

    Fingerprinting/ Background Check

    Fingerprints are required to do a background check with the State of Missouri which is required to become a volunteer with any youth-centered program at Midtown.

    Health Screening/ Screening

    It is now required that Midtown have a health screening on file for each volunteer as well as a TB Risk assessment.

    Midtown will provide a waiver for the cost associated with fingerprinting and health screening.  If you would like to cover these costs on your own please let Midtown staff know when you receive the email.

     

    A volunteer will not be able to start volunteering until these 2 processes have been completed.

  • Volunteer Waivers & Policies

  • I desire to work as a volunteer for Midtown Community Services and engage in the activities related to being a volunteer for a work project.

    I hereby voluntarily, execute this Volunteer Waiver under the following terms:

    I, the Volunteer, release and hold harmless Midtown Community Services and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer work with the Organization.

    I understand that this Waiver discharges Midtown Community Services from any liability or claim that I, the Volunteer, may have against Midtown Community Services with respect to bodily injury, personal injury, illness, death, or property damage that may result from my participation on the Midtown Community Services’ site.

    I also fully understand that Midtown Community Services does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage.

    I, the Volunteer, understand that I expressly waive any such claim for compensation or liability on the part of Midtown Community Services beyond what may be offered freely by the representative of Midtown Community Services in the event of such injury or medical expense.

    I hereby release Midtown Community Services from any claim whatsoever which arises or may arise in the future on account of any first aid treatment or other medical services that are conducted in connection with an emergency during my time with Midtown Community Services.

    I understand that my time with Midtown may include various activities that may be hazardous to me and I hereby expressly and specifically assume the risk of injury or harm in these activities and release Midtown Community Services from all liability for injury, illness, death, or property damage resulting from the activities of my time with Midtown Community Services.


    I grant unto Midtown Community Services all right, title, and interest in any and all photographic images and video or audio recordings that are made by Midtown Community Services during my work with Midtown Community Services, including, but not limited to, any royalties, proceeds, or other benefits that are derived from such photographs or recordings.

    I expressly agree that this Waiver is intended to be as broad and inclusive as permitted by the laws of the State of Missouri in the United States of America, and that this Waiver shall be governed by and interpreted in accordance with the laws of the State of Missouri. I agree that in the event that any clause or provision of this Waiver shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to enforceable.

  • I understand that since I may be working with minors, I will be subject regularly to a thorough background check inducing criminal history.

    I understand that any action inconsistent with Midtown Community Services’ Ethical Conduct for Employees & Volunteers Working with Minors or failure to take action mandated thereby may result in my removal from my position working with minors.

    I further understand that I may, as a condition of continuing my position, be required to participate in education and training.

  • Midtown Community Services must protect the confidentiality of information for all individuals served.

    Staff and volunteers have an ethical, moral and legal obligation, as required by federal and state regulations, to adhere to confidentiality policies of Midtown Community Services.

    A client’s right to privacy is of high priority. Any unauthorized disclosure of confidential information in written, electronic, or oral form is considered a violation of agency policy.

    Violations of this policy could result in monetary fines and disciplinary action, including possible termination of employment.

  • Midtown Community Services encourages its staff and volunteers to utilize technology to improve communication, maximize efficiency, and better serve the clients of Midtown Community Services.

    I understand that I am not to use them for my own personal, private or non-business matters to communicate personal, private, or non-business matters. I agree that I will not communicate anything that might be construed as harassment or offensive to others based on ethnicity, gender, disability, age, religion or national origin.

    By logging on to a Midtown workstation or technological device, opening email, sending or receiving information, logging on to the internet or by use of any of the software, I am agreeing to and understand that this technology has been approved by Midtown Community Services and is Midtown Community Services’ property. It is another tool for my use in business transactions and communications.
