Toddler Classes Registration Form

Classes will be held Saturdays from 10:30am-11:00am & 11:00am- 11:30am for ages 16 months - 2.5 years.

If your dancer is between ages 2.5 years and 3.5 years but not ready for our independent Mini class yet, they are still welcome to join our Toddler Classes.

Each 4 week session is $48 due by the first class of the session. 1 Adult must participate in class with their dancer.

If you register for a class and are unable to attend the session, you must notify us by email or phone.

Class will include songs, dancing & mat.

Payment is due by the first class of the session. Payment cannot be prorated for missed classes. Payments can be made by cash or checks, make checks payable to Contemporary Academy of Dance. Payments are non-refundable.

For public relation purposes photos and videos may be taken of you or your child. If you object to the use of these photos or videos you must notify Contemporary Academy of Dance in writing.
Liability Release
Contemporary Academy of Dance is not liable for any injury, damage or loss of property. I release Contemporary Academy of Dance from all liability, injury, damage or loss of property. I have read and agree to the payment terms, fees, and policies listed on our website.
