The licensed children’s services or kindergarten must collect the child’s enrolment information in this form, as required by the Education and Care Services. National Law Act 2010(National Law), the Education and Care Services National Regulations 2011 (National Regulations)  Please fill in all sections of this form.  If a section is not relevant, please put a line through it. This enrolment form is for your child’s journey in Kindergarten and only one enrolment form is required for the two year program.  It is assumed that all students enrolled in the Kindergarten program will attend for two years.

  • Authorised Nominees to collect your child from kindergarten

    Your consent is required for other people to collect your child from the kindergarten on your behalf. No adult can legally collect your child from kindergarten if they are not on this list. If you do not want to nominate anyone, please mark the first area with an X. This list may be changed throughout the year.
  • Authorised persons to be contacted in the event of an emergency and that the parents or guardians cannot be contacted.

    These people become authorised nominees. Authorised nominee means a person who has been given permission by a parent or family member to collect the child from the kindergarten. (See section 170(5) of the Law). In the event that the child is not collected from the kindergarten, or in the case of illness, accident, injury, trauma or emergency when the parents or guardians cannot be contacted, this list will also be used to arrange for someone to collect your child. This list may be changed throughout the year. You can write as above if applicable. At least one contact must be listed, this area cannot be left blank.
  • People other than parents or guardians who are authorised to give consent to medical treatment for your child or to authorise the administration of medication to your child.

    There may be times when your child has an accident, injury, trauma or illness and consent must be given to medically treat the child and / or administer medication to the child. Please note that every attempt will be made to reach the parents or guardians first. If you do not want to nominate anyone, please mark with an X. You can write as above if applicable.   • Childs Immunisation Record.

    All kindergartens are required by law to keep a record of the immunisation status of all children enrolled at the kindergarten.  Parent must provide  a copy of their child's current immunisatation statement to show that their immunisations are up to date. This certificate is provided by the Australian Immunisation Register, ph ( 1800 653 809) and is available from your myGov account.

  • In the case of anaphylaxis, you will be provided with a copy of the kindergarten's anaphylaxis management policy. You will be required to provide an individual medical management plan for your child signed by a medical practitioner. This will need to be attached to this enrolment form.

  • Declaration and consent to emergency medical treatment:

  • • declare that the information in this enrolment form is true and correct and will
    immediately inform the kindergarten in the event of any change to this information;
    • agree to collect or make arrangements for the collection of my child if they become unwell
    • consent to the staff of the kindergarten seeking, or where appropriate, administering, emergency
    medical treatment as is necessary and to reimburse any expenses incurred
    • consent to staff applying sunscreen as appropriate to all exposed skin of my child
    • consent to the kindergarten staff seeking medical treatment from a registered medical
    practitioner, hospital or ambulance service for my child;
    • agree to support the philosophy of the kindergarten;
    • agree to payment of  fees as invoiced by the Ballarat Steiner School and Kindergarten
    A term’s notice in writing is expected before a child is withdrawn from the kindergarten. If no notice is given, a terms fees will be charged and payable in lieu of notice. If your child is absent from kindergarten for an extended period (being one term or longer) – you are liable for half the tuition fees for the period of
    absence. Written notice of the period of absence from the parent/s must be lodged with the administration office.

    The Ballarat Steiner School and Kindergarten is aware of and implements the requirements of the State and National Privacy principles and obtains consent for uses and disclosures of personal information that may not be for the primary or secondary related purposes to the collection:


    The kindergarten collects personal information, including sensitive information about pupils and parents or guardians before and during the course of the student’s enrolment at the kindergarten. The primary purpose of collecting this information is to enable the kindergarten to provide duty of care to your child.

    1. Some of the information we collect is to satisfy the kindergarten’s legal obligations, particularly to enable the kindergarten to discharge its duty of care.
    2. Certain laws governing or relating to the operation of the kindergarten require that certain information is collected. These include Public Health and Child Protection laws.
    3. Health information about students is sensitive information within the terms of the National Privacy Principles under the Privacy Act. We ask you to provide medical reports about students from time to time.
    4. The kindergarten from time to time discloses personal and sensitive information to others for administrative and educational purposes. This includes to other kindergartens, schools, government departments, medical practitioners, and people providing services to the kindergarten, including specialist visiting teachers, sports teachers and volunteers.
    5. If we do not obtain the information referred to above, we may not be able to enroll or continue the enrolment of your child.
    6. Personal information collected regarding students is regularly disclosed to their parents or guardians. On occasions information such as academic and sporting achievements, student activities and other news is published in the school and kindergarten newsletters, magazines and on our website.
    7. Parents may seek access to personal information collected about them and their child by contacting the front office. Students may also seek access to personal information about them. However, there will be occasions when access is denied. Such occasions would include where access would have an unreasonable impact on the privacy of others, where access may result in a breach of the kindergarten’s duty of care to the student, or where students have provided information in confidence.
    8. As you may know the kindergarten from time to time engages in fundraising activities. Information received from you may be used by the kindergarten or an agent of the kindergarten to make an appeal to you.
