As part of Enrolment at our Service, we require acceptance of the following items in order to receive Government Funding on your behalf. Acceptance of these items will be via a Complying Written Agreement (CWA) for Child Care Subsidy (CCS) purposes. The CWA can be viewed in your child's profile in Xplor.

I understand that Sherwood State School OSHC will notify me of the days my child/ren have been allocated bookings. I understand if they cannot be confirmed they will be placed on a Waitlist and I will be informed should a spot become available. I understand I must contact Sherwood State School OSHC if I wish to change/cancel bookings and give the required notice to avoid charges (refer to Cancellation Policy in the Family Handbook).
