FIRST HOLY COMMUNION & RECONCILIATION REGISTRATION FORM

Dear Parents/Guardians,

We warmly welcome your family to register for your child’s sacramental preparation.

We offer preparation to families with children: in year 3 or above in the current academic year; and who attend Mass at Our Lady and St Dominic.

Please complete the online form or hand in a copy to the Parish Office no later than the 29th of September 2023.

The suggested donation towards the costs of the First Reconciliation and First Holy Communion Preparation Programme 2023-2024 is £30 per child.

Mass attendance is expected on Weekends
Mass times are:- Saturday 17:30; Sunday 9am and 10:30

Parents’ Meetings 8pm in hall or church.

Children’s Meetings are on Tuesdays 4:45-6:15 pm in the Church Hall. Please refer to the First Communion Programme Dates published on the website.

PARENTAL CONSENT

I give permission for my child, named above, to begin formation for the reception of his/her First Holy Communion and to all the events contained in the formation programme. As the first educator of my child in the ways of Faith, I guarantee that I will assist him/her in the preparations and bring him/her to Mass every Sunday and Holydays of Obligation.

In the unlikely event of any accident, injury or illness, I authorize Fr Anthony Lobo and/or his assistants to administer and/or to authorise, in my name, any first aid treatment, judged to be in my child’s best interests, until I or another member of the family arrive to take charge of the situation. I draw your attention to the Medical Alert information entered below (If applicable) and will inform you in writing if there are any changes to this information which may affect my child’s health.
